If you wish for your child to be given medicine in school, please can you download the following form, fill it in then email it back to the school office.

If your child requires an emergency inhaler or epi-pen, please download the following forms and send back to the school office once completed.

We are unable to give medicines without page one of this consent form being completed and signed.
